However, the user can include an "expiration date," after which the information is no longer usable by anyone who may have a copy of it, even the creator. The Vanish approach was found to be vulnerable to a Sybil attack and thus insecure by a team called Unvanish from the University of Texas, University of Michigan, and Princeton. == Theory == Vanish acts by automating the encryption of information entered by the user with an encryption key that is unknown to the user. Along with the information the user enters, the user also enters metadata concerning how long the information should remain available. The system then encrypts the information but does not store either the encryption key or the original information. Instead, it breaks up the decryption key into smaller components that are disseminated across distributed hash tables, or DHTs, via the Internet. The DHTs refresh information within their nodes on a set schedule unless configured to make the information persistent. The time delay entered by the user in the metadata controls how long the DHTs should allow the information to persist, but once that time period is over, the DHTs will reuse those nodes, making the information about the decryption stored irretrievable. As long as the decryption key may be reassembled from the DHTs, the information is retrievable. However, once the period entered by the user has lapsed, the information is no longer recoverable, as the user never possessed the decryption key. == Implementation == Vanish currently exists as a Firefox plug-in which allows a user to enter text into either a standard Gmail email or Facebook message and choose to send the message via Vanish. The message is then encrypted and sent via the normal networking pathways through the cloud to the recipient. The recipient must have the same Firefox plug-in to decrypt the message. The plugin accesses BitTorrent DHTs, which have 8-hour lifespans. This means the user may select an expiration date for the message in increments of 8 hours. So, on March 2, 2022, the "Lviv Siren" bot was created, which reported on air alarms in Lviv on Twitter. Later, the idea arose to monitor alarms all over Ukraine and display them on a map. However, the lack of a single official source reporting alarms made this task much more difficult. On March 15, 2022, the Ajax Systems company announced the creation of the official Telegram channel "Air Alarm". This channel receives signals from the "Air Alarm" application and instantly publishes messages about the start and end of alarms in different regions of Ukraine. This immediately solved the problem with the source of information and gave impetus to the further implementation of the project. On March 22, 2022, the first version of the "Air Alarm Map" website was published, located on the war.ukrzen.in.ua domain. The map quickly gained popularity in social networks. It, like several other similar projects, began to be widely distributed by the mass media: Suspilne, Novyi Kanal, UNIAN, DW, Fakty ICTV, Vikna TV, Ukrainian Radio, STB, Espresso, dev.ua, itc.ua and state bodies: Center for Countering Disinformation at the National Security and Defense Council of Ukraine, Verkhovna Rada of Ukraine, Khmelnytska OVA, etc. On April 8, 2022, the site moved to the alerts.in.ua domain, where it is still available today. On August 25, 2022, the service began monitoring local official channels in addition to the main "Air Alarm". On September 11, 2022, the English version of the site was published. On March 22, 2023, its own Android application was published. The project is actively developing and has its own community. == Description == The main part of the site is a map of Ukraine, on which the regions where an air alert or other threats have been declared are highlighted in real time. As of October 16, 2022, 5 types of threats are supported: Air alarm. The threat of artillery fire. The threat of street fighting. Chemical threat. Nuclear threat. Additionally, based on media reports, information is published about other dangerous events, such as explosions, demining, etc. On the site, you can view the history of announced alarms with links to sources. Alarm statistics for different time periods are also available. For developers, there is an API that allows you to develop your own services based on information about declared alarms. The site is available in Ukrainian, English, Polish and Japanese. == Use == The map is used by: To monitor the situation in the country and the region. To illustrate the alarms announced in the mass media: TSN, Ukrainian truth, Channel 24, Suspilne, RBC Ukraine, Gromadske, Glavkom. As a map of alarms in mobile applications, there is Alarm and AirAlert. As an API for its services, including alternative alarm maps, Telegram, Viber channels, Discord bots, IoT projects, etc. == Statistics == 89.5% of users use the map from a mobile phone, 10% from a PC and 1% from a tablet.     Texture filtering

    In computer graphics, texture filtering or texture smoothing is the method used to determine the texture color for a texture mapped pixel, using the colors of nearby texels (ie. pixels of the texture). Filtering describes how a texture is applied at many different shapes, size, angles and scales. Depending on the chosen filter algorithm, the result will show varying degrees of blurriness, detail, spatial aliasing, temporal aliasing and blocking. Depending on the circumstances, filtering can be performed in software (such as a software rendering package) or in hardware, eg. with either real time or GPU accelerated rendering circuits, or in a mixture of both. For most common interactive graphical applications, modern texture filtering is performed by dedicated hardware which optimizes memory access through memory cacheing and pre-fetch, and implements a selection of algorithms available to the user and developer. There are two main categories of texture filtering: magnification filtering and minification filtering. Depending on the situation, texture filtering is either a type of reconstruction filter where sparse data is interpolated to fill gaps (magnification), or a type of anti-aliasing (AA) where texture samples exist at a higher frequency than required for the sample frequency needed for texture fill (minification). There are many methods of texture filtering, which make different trade-offs between computational complexity, memory bandwidth and image quality. == The need for filtering == During the texture mapping process for any arbitrary 3D surface, a texture lookup takes place to find out where on the texture each pixel center falls. For texture-mapped polygonal surfaces composed of triangles typical of most surfaces in 3D games and movies, every pixel (or subordinate pixel sample) of that surface will be associated with some triangle(s) and a set of barycentric coordinates, which are used to provide a position within a texture. Such a position may not lie perfectly on the "pixel grid," necessitating some function to account for these cases. In other words, since the textured surface may be at an arbitrary distance and orientation relative to the viewer, one pixel does not usually correspond directly to one texel. Some form of filtering has to be applied to determine the best color for the pixel. Insufficient or incorrect filtering will show up in the image as artifacts (errors in the image), such as 'blockiness', jaggies, or shimmering. There can be different types of correspondence between a pixel and the texel/texels it represents on the screen. These depend on the position of the textured surface relative to the viewer, and different forms of filtering are needed in each case. Given a square texture mapped on to a square surface in the world, at some viewing distance the size of one screen pixel is exactly the same as one texel. Closer than that, the texels are larger than screen pixels, and need to be scaled up appropriately — a process known as texture magnification. Farther away, each texel is smaller than a pixel, and so one pixel covers multiple texels. In this case an appropriate color has to be picked based on the covered texels, via texture minification. Graphics APIs such as OpenGL allow the programmer to set different choices for minification and magnification filters. Note that even in the case where the pixels and texels are exactly the same size, one pixel will not necessarily match up exactly to one texel. It may be misaligned or rotated, and cover parts of up to four neighboring texels. Hence some form of filtering is still required. == Mipmapping == Mipmapping is a standard technique used to save some of the filtering work needed during texture minification. It is also highly beneficial for cache coherency - without it the memory access pattern during sampling from distant textures will exhibit extremely poor locality, adversely affecting performance even if no filtering is performed. During texture magnification, the number of texels that need to be looked up for any pixel is always four or fewer; during minification, however, as the textured polygon moves farther away potentially the entire texture might fall into a single pixel. This would necessitate reading all of its texels and combining their values to correctly determine the pixel color, a prohibitively expensive operation. Mipmapping avoids this by prefiltering the texture and storing it in smaller sizes down to a single pixel. As the textured surface moves farther away, the texture being applied switches to the prefiltered smaller size. Different sizes of the mipmap are referred to as 'levels', with Level 0 being the largest size (used closest to the viewer), and increasing levels used at increasing distances. == Filtering methods == This section lists the most common texture filtering methods, in increasing order of computational cost and image quality. === Nearest-neighbor interpolation === Nearest-neighbor interpolation is the simplest and crudest filtering method — it simply uses the color of the texel closest to the pixel center for the pixel color. While simple, this results in a large number of artifacts - texture 'blockiness' during magnification, and aliasing and shimmering during minification. This method is fast during magnification but during minification the stride through memory becomes arbitrarily large and it can often be less efficient than MIP-mapping due to the lack of spatially coherent texture access and cache-line reuse. === Nearest-neighbor with mipmapping === This method still uses nearest neighbor interpolation, but adds mipmapping — first the nearest mipmap level is chosen according to distance, then the nearest texel center is sampled to get the pixel color. This reduces the aliasing and shimmering significantly during minification but does not eliminate it entirely. In doing so it improves texture memory access and cache-line reuse through avoiding arbitrarily large access strides through texture memory during rasterization. This does not help with blockiness during magnification as each magnified texel will still appear as a large rectangle. === Linear mipmap filtering === Less commonly used, OpenGL and other APIs support nearest-neighbor sampling from individual mipmaps whilst linearly interpolating the two nearest mipmaps relevant to the sample. === Bilinear filtering === In Bilinear filtering, the four nearest texels to the pixel center are sampled (at the closest mipmap level), and their colors are combined by weighted average according to distance. This removes the 'blockiness' seen during magnification, as there is now a smooth gradient of color change from one texel to the next, instead of an abrupt jump as the pixel center crosses the texel boundary. Bilinear filtering for magnification filtering is common. When used for minification it is often used with mipmapping; though it can be used without, it would suffer the same aliasing and shimmering problems as nearest-neighbor filtering when minified too much. For modest minification ratios, however, it can be used as an inexpensive hardware accelerated weighted texture supersample. The Nintendo 64 used an unusual version of bilinear filtering where only three pixels are used known as 3-point texture filtering, instead of four due to hardware optimization concerns. This introduces a noticeable "triangulation bias" in some textures. === Trilinear filtering === Trilinear filtering is a remedy to a common artifact seen in mipmapped bilinearly filtered images: an abrupt and very noticeable change in quality at boundaries where the renderer switches from one mipmap level to the next. Trilinear filtering solves this by doing a texture lookup and bilinear filtering on the two closest mipmap levels (one higher and one lower quality), and then linearly interpolating the results. This results in a smooth degradation of texture quality as distance from the viewer increases, rather than a series of sudden drops. Of course, closer than Level 0 there is only one mipmap level available, and the algorithm reverts to bilinear filtering. === Anisotropic filtering === Anisotropic filtering is the highest quality filtering available in current consumer 3D graphics cards. Simpler, "isotropic" techniques use only square mipmaps which are then interpolated using bi– or trilinear filtering. (Isotropic means same in all directions, and hence is used to describe a system in which all the maps are squares rather than rectangles or other quadrilaterals.) When a surface is at a high angle relative to the camera, the fill area for a texture will not be approximately square. Consider the common case of a floor in a game: the fill area is far wider than it is tall. In this case, none of the square maps are a good fit.     A gonioreflectometer is a device for measuring a bidirectional reflectance distribution function (BRDF). The device consists of a light source illuminating the material to be measured and a sensor that captures light reflected from that material. The light source should be able to illuminate and the sensor should be able to capture data from a hemisphere around the target. The hemispherical rotation dimensions of the sensor and light source are the four dimensions of the BRDF. The 'gonio' part of the word refers to the device's ability to measure at different angles. Several similar devices have been built and used to capture data for similar functions. Most of these devices use a camera instead of the light intensity-measuring sensor to capture a two-dimensional sample of the target. Examples include: a spatial gonioreflectometer for capturing the SBRDF (McAllister, 2002). a camera gantry for capturing the light field (Levoy and Hanrahan, 1996). an unnamed device for capturing the bidirectional texture function (Dana et al., 1999).

    Screen space ambient occlusion

    Screen space ambient occlusion (SSAO) is a computer graphics technique for efficiently approximating the ambient occlusion effect in real time. It was developed by Vladimir Kajalin while working at Crytek and was used for the first time in 2007 by the video game Crysis, also developed by Crytek. == Implementation == The algorithm is implemented as a pixel shader, analyzing the scene depth buffer which is stored in a texture. For every pixel on the screen, the pixel shader samples the depth values around the current pixel and tries to compute the amount of occlusion from each of the sampled points. In its simplest implementation, the occlusion factor depends only on the depth difference between sampled point and current point. Without additional smart solutions, such a brute force method would require about 200 texture reads per pixel for good visual quality. This is not acceptable for real-time rendering on current graphics hardware. In order to get high quality results with far fewer reads, sampling is performed using a randomly rotated kernel. The kernel orientation is repeated every N screen pixels in order to have only high-frequency noise in the final picture. In the end this high frequency noise is greatly removed by a NxN post-process blurring step taking into account depth discontinuities (using methods such as comparing adjacent normals and depths). Such a solution allows a reduction in the number of depth samples per pixel to about 16 or fewer while maintaining a high quality result, and allows the use of SSAO in soft real-time applications like computer games. Compared to other ambient occlusion solutions, SSAO has the following advantages: Independent from scene complexity. No data pre-processing needed, no loading time and no memory allocations in system memory. Works with dynamic scenes. Works in the same consistent way for every pixel on the screen. No CPU usage – it can be executed completely on the GPU. May be easily integrated into any modern graphics pipeline. SSAO also has the following disadvantages: Rather local and in many cases view-dependent, as it is dependent on adjacent texel depths which may be generated by any geometry whatsoever. Hard to correctly smooth/blur out the noise without interfering with depth discontinuities, such as object edges (the occlusion should not "bleed" onto objects). Because SSAO operates only on the current depth buffer, it can miss occluding geometry that is not rasterized into the z-buffer and may produce undersampling-related artifacts.

    A datasource or DataSource is a name given to the connection set up to a database from a server. The name is commonly used when creating a query to the database. The data source name (DSN) need not be the same as the filename for the database. For example, a database file named friends.mdb could be set up with a DSN of school. Then DSN school would be used to refer to the database when performing a query. == Sun's version of DataSource [1] == A factory for connections to the physical data source that this DataSource object represents. An alternative to the DriverManager facility, a DataSource object is the preferred means of getting a connection. An object that implements the DataSource interface will typically be registered with a naming service based on the Java Naming and Directory Interface (JNDI) API. The DataSource interface is implemented by a driver vendor. There are three types of implementations: Basic implementation — produces a standard Connection object Connection pooling implementation — produces a Connection object that will automatically participate in connection pooling. This implementation works with a middle-tier connection pooling manager. Distributed transaction implementation — produces a Connection object that may be used for distributed transactions and almost always participates in connection pooling. This implementation works with a middle-tier transaction manager and almost always with a connection pooling manager. A DataSource object has properties that can be modified when necessary. For example, if the data source is moved to a different server, the property for the server can be changed. The benefit is that because the data source's properties can be changed, any code accessing that data source does not need to be changed. A driver that is accessed via a DataSource object does not register itself with the DriverManager. Rather, a DataSource object is retrieved through a lookup operation and then used to create a Connection object. With a basic implementation, the connection obtained through a DataSource object is identical to a connection obtained through the DriverManager facility. == Sun's DataSource Overview [2] == A DataSource object is the representation of a data source in the Java programming language. In basic terms, a data source is a facility for storing data. It can be as sophisticated as a complex database for a large corporation or as simple as a file with rows and columns. A data source can reside on a remote server, or it can be on a local desktop machine. Applications access a data source using a connection, and a DataSource object can be thought of as a factory for connections to the particular data source that the DataSource instance represents. The DataSource interface provides two methods for establishing a connection with a data source. Using a DataSource object is the preferred alternative to using the DriverManager for establishing a connection to a data source. They are similar to the extent that the DriverManager class and DataSource interface both have methods for creating a connection, methods for getting and setting a timeout limit for making a connection, and methods for getting and setting a stream for logging. Their differences are more significant than their similarities, however. Unlike the DriverManager, a DataSource object has properties that identify and describe the data source it represents. Also, a DataSource object works with a Java Naming and Directory Interface (JNDI) naming service and can be created, deployed, and managed separately from the applications that use it. A driver vendor will provide a class that is a basic implementation of the DataSource interface as part of its Java Database Connectivity (JDBC) 2.0 or 3.0 driver product. What a system administrator does to register a DataSource object with a JNDI naming service and what an application does to get a connection to a data source using a DataSource object registered with a JNDI naming service are described later in this chapter. Being registered with a JNDI naming service gives a DataSource object two major advantages over the DriverManager. First, an application does not need to hardcode driver information, as it does with the DriverManager. A programmer can choose a logical name for the data source and register the logical name with a JNDI naming service. The application uses the logical name, and the JNDI naming service will supply the DataSource object associated with the logical name. The DataSource object can then be used to create a connection to the data source it represents. The second major advantage is that the DataSource facility allows developers to implement a DataSource class to take advantage of features like connection pooling and distributed transactions. Connection pooling can increase performance dramatically by reusing connections rather than creating a new physical connection each time a connection is requested. The ability to use distributed transactions enables an application to do the heavy duty database work of large enterprises. Although an application may use either the DriverManager or a DataSource object to get a connection, using a DataSource object offers significant advantages and is the recommended way to establish a connection. Since 1.4 Since Java EE 6 a JNDI-bound DataSource can alternatively be configured in a declarative way directly from within the application. This alternative is particularly useful for self-sufficient applications or for transparently using an embedded database. == Yahoo's version of DataSource [3] == A DataSource is an abstract representation of a live set of data that presents a common predictable API for other objects to interact with. The nature of your data, its quantity, its complexity, and the logic for returning query results all play a role in determining your type of DataSource. For small amounts of simple textual data, a JavaScript array is a good choice. If your data has a small footprint but requires a simple computational or transformational filter before being displayed, a JavaScript function may be the right approach. For very large datasets—for example, a robust relational database—or to access a third-party webservice you'll certainly need to leverage the power of a Script Node or XHR DataSource.

    Reverse correlation technique

    The reverse correlation technique is a data driven study method used primarily in psychological and neurophysiological research. This method earned its name from its origins in neurophysiology, where cross-correlations between white noise stimuli and sparsely occurring neuronal spikes could be computed quicker when only computing it for segments preceding the spikes. The term has since been adopted in psychological experiments that usually do not analyze the temporal dimension, but also present noise to human participants. In contrast to the original meaning, the term is here thought to reflect that the standard psychological practice of presenting stimuli of defined categories to the participants is "reversed": Instead, the participant's mental representations of categories are estimated from interactions of the presented noise and the behavioral responses. It is used to create composite pictures of individual and/or group mental representations of various items (e.g. faces, bodies, and the self) that depict characteristics of said items (e.g. trustworthiness and self-body image). This technique is helpful when evaluating the mental representations of those with and without mental illnesses. == Terms == This technique utilizes spike-triggered average to explain what areas of signal and noise in an image are valuable for the given research question. Signal is information used to produce objects of value that help explain and connect the world around us. Noise is commonly referred to as unwanted signal that obscures the information that the signal is trying to present. Most importantly for reverse correlation studies, noise is randomly varying information. To determine the areas of importance using reverse correlation, noise is applied to a base image and then evaluated by observers. A base image is any image void of noise that relates to the research question. A base image that has noise superimposed on top is the stimuli that is presented to and evaluated by participants. Each time a new set of stimuli is presented to a participant, this is known as a trial. After a participant has responded to hundreds to thousands of trials, a researcher is ready to create a classification image. A classification image (abbreviated as "CI" in some studies) is a single image that represents the average noise patterns in the images selected by participants. A classification image can also be computed for groups by averaging the individuals’ classification images. These classification images are what researchers use to interpret the data and draw conclusions. As a whole, the reverse correlation method is a process that results in a composite image (from an individual or group) that can be used to estimate and interpret mental representations. == Basic study layout == The reverse correlation method is typically executed as an in-lab computer experiment. This method follows four broad steps. Each of the following steps are described in greater detail below. After creating a research question and determining that the reverse correlation method is the most suitable technique to answer the question, a researcher must (1) design randomly varying stimuli. After the stimuli have been prepared, a researcher should (2) collect data from participants who will see and respond to approximately 300 -1,000 trials. Each trial will either consist of one or two images (side by side) derived from the same base image with noise superimposed on top. Participant responses will depend on the chosen study design; if a researcher presents only one image at a time, participants rate the image on a 4pt scale, but when two images are shown, the participant is asked to choose which best aligns with the given category (e.g. choose the image that looks the most aggressive). Once all of the data is collected, the researcher will (3) compute classification images for each participant and using those images compute group classification images. Finally, with the classification images available, the researcher will (4) evaluate the images and draw conclusions about their results. === Step 1: making stimuli === When designing the stimuli for a reverse correlation study, the two primary factors that one should consider are (1) the base image and (2) the noise that will be used. While not all bases are images per se, the majority are and for this reason the base is typically referred to as a base image. The base image should represent whatever the research question is addressing. For example, if you are interested in peoples’ mental representations of Chinese people, it would not make sense to use a base image of a Spanish or Caucasian person. Again, if you are interested in the mental representations of male vocal patterns, it would make the most sense to use a base vocal pattern that has been produced by a male. Having a base is important because it provides a kind of anchor for participants to work from. When there is no base image, the number of trials that are required increases dramatically, thus making it harder to collect data. While there are studies that have excluded a base image, (e.g. the S study), for more elaborate and nuanced research questions, it is important to have a base image that is a fair representation of what participants are being asked to categorize. Photographs of faces are generally the most popular base image. Although the reverse correlation method is capable of investigating a wide variety of research questions, the most common application of the method is for evaluating faces on a single trait. Reverse correlation studies that address evaluations of the face are sometimes referred to as being a face space reverse correlation model (FSRCM). Thankfully, there are existing databases for face images of varying demographics and emotion that work well as base images. The reverse correlation method can also be used to help researchers identify what areas of an image (e.g. the areas on the face) have diagnostic value. In order to identify these areas of value, researchers start by minimizing the space a participant can pull information from. By imposing a “mask” on an image (e.g. blur an image while leaving random areas un-blurred), this reduces the information individuals might see, and forces them to focus on certain areas. Then, if/when participants are able to correctly identify an image with a trait repeatedly, we can draw conclusions about what areas have diagnostic value. While faces and visual stimuli are the most popular, this is not the only stimuli that can be used in a reverse correlation study. This method was originally designed for auditory stimuli which allows researchers to investigate how perceivers interpret auditory information and create trait based attributions to different sound patterns. For example, by segmenting a vocal recording of a single word (total sound time 426 ms) into six segments (71 ms each), and varying each segment's pitch using Gaussian distributions, researchers were able to uncover what vocal patterns people associated with certain traits. Specifically, this study investigated how listeners rated sound clips of the word “really” as sounding more interrogative (i.e. like the more common reverse correlation studies this study had participants listen to two sound clips per trial, choose which fit the category the best, and then created an average of the pitch contours). Beyond face and auditory perception, research utilizing the reverse correlation method has expanded to investigate how individuals see three-dimensional objects in images with noise (but no signal). After selecting your base image, regardless of what the image is, it is helpful to apply a Gaussian blur to smooth noise in the image. While noise will be applied later, it is helpful to reduce existing noise in the photo before applying your chosen noise. There are three primary choices when it comes to noise: white noise, sine-wave noise, and Gabor noise. The latter two of these constrain the configurations that the noise can have, and because of this white noise is usually the most commonly used. Regardless of the type of noise that is chosen, it is crucial that the noise randomly varies. === Step 2: data collection === Once the stimuli for the study has been developed, the researcher must make a few decisions before actually collecting the data. The researcher must come to a conclusion on how many stimuli will be presented at a time and how many trials the participants will see. In terms of stimuli presentation, a researcher can choose from either a 2-Image Forced Choice (2IFC) or a 4-Alternative Forced Choice (4AFC). The 2IFC presents two images at once (side by side) and requires participants to choose between the two on a specified category (e.g. which image looks the most like a male).     Medical data, including patients' identity information, health status, disease diagnosis and treatment, and biogenetic information, not only involve patients' privacy but also have a special sensitivity and important value, which may bring physical and mental distress and property loss to patients and even negatively affect social stability and national security once leaked. However, the development and application of medical AI must rely on a large amount of medical data for algorithm training, and the larger and more diverse the amount of data, the more accurate the results of its analysis and prediction will be. However, the application of big data technologies such as data collection, analysis and processing, cloud storage, and information sharing has increased the risk of data leakage. In the United States, the rate of such breaches has increased over time, with 176 million records breached by the end of 2017. By 2024, the U.S. Department of Health and Human Services reported 725 large healthcare data breaches affecting approximately 275 million individual records in a single year, marking a significant escalation in both the frequency and scale of incidents. == Black market for health data == In February 2015 an NPR report claimed that organized crime networks had ways of selling health data in the black market. Among reported breaches of medical information in the United States networked information systems accounted for the largest number of records breached. There are many data breaches happening in the US health care system, among business associates of the health care providers that continuously gain access to patients' data. == List of data breaches == In February 2024, a ransomware attack on Change Healthcare, a subsidiary of UnitedHealth Group, compromised the protected health information of approximately 100 million individuals, making it the largest healthcare data breach in United States history. The attack disrupted claims processing for healthcare providers nationwide for several weeks. In May 2024, MediSecure suffered a cyberattack involving ransomware in Australia. In May 2021, the Health Service Executive in the Republic of Ireland was the victim of a cyberattack involving ransomware, in the Health Service Executive cyberattack, with admission records and test results present in a sample of the data reviewed by the Financial Times. In October 2018, the Centers for Medicare and Medicaid Services in the US reported that around 75,000 individual records had been affected by a data breach that took place through the ACA Agent and Broker Portal. In 2018, Social Indicators Research published the scientific evidence of 173,398,820 (over 173 million) individuals affected in USA from October 2008 (when the data were collected) to September 2017 (when the statistical analysis took place). In 2015, Anthem Inc. lost data for 37 million people in the Anthem medical data breach In 2014 4.5 million people using Complete Health Systems had their data stolen In 2013-14 1 million people using Montana Department of Public Health and Human Services had their data stolen In 2013 4 million people using Advocate Health and Hospitals Corporation had their data stolen In 2011 4.9 million users of Tricare services had their data stolen due to an employee error by Science Applications International Corporation In 2011 1.9 million people using Health Net had their data stolen In 2011 1 million people using Nemours Foundation had their data stolen In 2010 6800 people using New York-Presbyterian Hospital and Columbia University Medical Center had their data breached. In response, those organizations agreed to pay the United States Department of Health and Human Services a US$4.8 million dollar fine. In 2009 1 million people using BlueCross BlueShield of Tennessee had their data stolen == Regulation == In the United States, the Health Insurance Portability and Accountability Act and Health Information Technology for Economic and Clinical Health Act require companies to report data breaches to affected individuals and the federal government. Under the HIPAA Breach Notification Rule, covered entities must notify affected individuals without unreasonable delay and no later than 60 days after discovering a breach of unsecured protected health information. Breaches affecting 500 or more individuals must also be reported to the HHS Secretary and to prominent media outlets serving the affected state or jurisdiction within the same timeframe; HHS publicly lists these larger breaches on its breach portal, commonly known as the "wall of shame." Breaches affecting fewer than 500 individuals are reported to HHS annually, no later than 60 days after the end of the calendar year in which they were discovered. Health Information Privacy Health Insurance Portability and Accountability Act of 1996 (HIPAA). - 45 CFR Parts 160 and 164, Standards for Privacy of Individually Identifiable Health Information and Security Standards for the Protection of Electronic Protected Health Information. HIPAA includes provisions designed to save health care businesses money by encouraging electronic transactions, as well as regulations to protect the security and confidentiality of patient information. The Privacy Rule became effective April 14, 2001, and most covered entities (health plans, health care clearinghouses, and health care providers that conduct certain financial and administrative transactions electronically) had until April 2003 to comply. This security provision became effective April 21, 2003. The Health Insurance Portability and Accountability Act (HIPAA) is the baseline set of federal regulations governing medical information. It does three things: i. i. i.Establish a structure for how personal health information is disclosed and establish the rights of individuals with respect to health information; ii.Specify security standards for the retention and transmission of electronic patient information; iii.Need a common format and data structure for the electronic exchange of health information. California-Specific Laws California’s medical privacy laws, primarily the Confidentiality of Medical Information Act (CMIA), the data breach sections of the Civil Code, and sections of the Health and Safety Code, provide HIPAA-like protections, although the terminology is different. HIPAA establishes a federal "minimum standard" that applies where there are gaps in California law, and HIPAA also specifies that stricter state laws will override or supersede HIPAA. California's health care privacy laws apply to providers who provide personal health records (PHR), while HIPAA only applies when the provider providing the PHR is a business associate of a covered entity. Federal law does not grant individuals the right to file a lawsuit in the event of a data breach (only the Attorney General can file a lawsuit), but California law does. This means that California law sets a higher standard for medical privacy, and that individuals in California enjoy stronger legal protections and more ways to hold entities that violate their medical privacy accountable. In the UK, the legal framework for how patient data is cared for and processed is the Data Protection Act 2018 (DPA), which incorporates the EU General Data Protection Regulation (GDPR) into law, and the common law duty of confidentiality (CLDC). The data protection legislation requires that the collection and processing of personal data be fair, lawful and transparent. This means that the collection and processing of data as defined by data protection legislation must always have a valid lawful basis and must also meet the requirements of the CLDC. In the China, Article 18 of the "National Health Care Big Data Standards, Security and Services Management Measures (for Trial Implementation)" (National Health Planning and Development (2018) No. 23) promulgated by the National Health Care Commission in 2018 states, "The responsible unit shall adopt measures such as data classification, important data backup, and encryption authentication to guarantee the security of health care big data." However, the scope and definition of important data are not covered. Although the "Information Security Technology-Healthcare Data Security Guide" (the "Guide") issued by the National Standardization Committee also proposes that important data should be evaluated and approved in accordance with the regulations, there is likewise no definition of the connotation and definition of important data.
